IMHO, the religious exemption should be nothing more than a checkbox.  Acknowledging sky wizards and other nonsense legitimizes religion, which are really just fantasies and delusions.

Also… the highlighted below is wicked stupid.  As man-made things, we know exactly what is in the vaccines.  We have the technology to identify, to the molecular or even atomic level, what is in the vaccine, and we have a list of the things that went into the vaccines.  There is no magic here.  Nothing but a list of ingredients is required.  Nothing needs to be “proven”:
